胰高血糖素是一种由29种氨基酸组成的多肽，由胰岛的α细胞分泌。它在胰腺α细胞中经过组织特异性处理形成胰高血糖素，在肠细胞中形成GLP-1和GLP-2。
胰高血糖素在糖尿病中的作用是复杂的，在某些情况下存在争议。然而，胰高血糖素在优化糖尿病管理方面的重要贡献越来越受到重视。胰高血糖素靶向治疗的不断发展突出了其在降糖药物中的作用。
胰高血糖素是一种反调节激素，刺激肝脏葡萄糖的产生，以避免低血糖。它在肾脏、心脏、胃肠道、脂肪组织、大脑和脾脏中也发现了胰高血糖素结合位点。胰高血糖素的分泌受到多种因素的调控，包括血糖水平、胰岛素、锌和伽马氨基丁酸等。
在正常生理中，胰高血糖素的分泌受到细胞分泌产物胰岛素、锌和伽马氨基丁酸(GABA)的抑制。当血糖水平下降时，胰岛细胞减少这些产品的生产，从而消除了它们对胰高血糖素分泌的抑制作用。这一机制被称为“关闭”假说，最终导致血糖过低时胰高血糖素分泌增加。
生长抑素由胰岛的δ细胞和GLP-1产生，也抑制胰高血糖素的分泌。
在1型糖尿病(T1DM)和2型糖尿病(T2DM)中，血浆胰高血糖素水平都过高。胰高血糖素作为一种促进肝脏葡萄糖生成的调节激素，在正常生理状态下可预防低血糖。然而，在糖尿病患者中，胰高血糖素分泌可能不受控制，这导致了葡萄糖稳态的问题。因此，抑制胰高血糖素的分泌或作用成为一些治疗糖尿病的方法，而胰高血糖素特异性靶向治疗也显示出良好的效果。
